Public health design – water, hot water and drainage for prime properties
In MEP engineering, public health has nothing to do with medicine or hospitals. It is the discipline that designs a building's water supply, hot-water generation, above-ground drainage and sanitary systems – everything that brings clean water to an outlet at the right pressure and temperature, and carries used water away quietly and reliably. The name is inherited from the sanitation engineering that made cities habitable; on a modern project it is simply the wet-services counterpart to the mechanical and electrical scopes.

The starting point
The problem: water and drainage treated as plumbing, not engineering
Water and drainage are the services a client experiences most directly and thinks about least – until a shower runs weak when a second bathroom is in use, a tap takes an age to run hot, or a soil stack drums through a bedroom wall at night. Every one of those outcomes is decided long before anyone chooses a tap. Incoming mains that were never surveyed, hot-water storage guessed rather than calculated, drainage routes improvised around a finished structure, plant squeezed into whatever room was left over – each is a strategy decision that defaulted to chance because nobody owned it early enough.
The difficulty is compounded on prime projects because the demands are higher and the tolerance for compromise is lower. Multiple bathrooms drawing simultaneously, body jets and oversized shower heads, steam rooms and spas, kitchens with several water-fed appliances – all served through a building where risers are contested, ceiling zones are shallow and drainage falls must thread through structure that was designed for other priorities. Without a public health engineer at the table, these conflicts surface on site, where the answers are expensive.
How AVC works
The AVC approach: strategy first, then detail
Every public health design we produce starts with two questions: what does the building demand, and what can the supply deliver? Demand is assessed room by room – outlets, simultaneity, hot-water draw – and the incoming supply is surveyed rather than assumed, because mains pressure and flow vary from street to street and decide whether the building needs boosted cold water, break tanks and pumps, or can run directly from the main. From that foundation come the strategic decisions: how hot water is generated and stored, how secondary circulation keeps distant outlets running hot without waste, where the stacks fall, and how much plant space the whole strategy honestly requires.
The detail then follows the strategy. Pipework is sized by calculation, drainage routes and falls are resolved against structure in the coordination model, stacks are kept away from the rooms that deserve quiet, and plant rooms are laid out for maintenance access rather than just for the equipment. Because AVC also delivers public health installations, the design is written by people who will stand behind it on site – schedules, schematics and commissioning requirements produced to be built from, not merely tendered against.

Why early involvement matters
The decisions that make or break a public health design – riser locations, plant space, floor build-ups that can accept drainage falls, where the incoming main enters – are all fixed early in the architectural design, often with nobody asking where the water will arrive or the waste will go. Once the structure is committed, a stack that needs to move or a plant room that proves too small is no longer a drawing revision; it is core drilling, redecoration and a programme claim.
Brought in at concept or developed design, we can test the incoming supply, size the plant honestly and reserve the routes while every option is still open. We join projects at later stages too, and often do – but the earlier the water and drainage strategy is owned, the fewer compromises the finished building has to live with.

What does public health mean in building services?+
It is the engineering discipline covering a building's water supply, hot-water generation, above-ground drainage and sanitary systems. The name comes from the sanitation engineering heritage of the profession and has nothing to do with medicine – in an MEP team, the public health engineer is the specialist in water and drainage.
Why does water pressure need a strategy on a London property?+
Because mains performance varies from street to street and cannot simply be assumed, while the demands of a prime property – several bathrooms in simultaneous use, generous shower fittings, water-fed appliances – are often well beyond what the incoming main will support unaided. The strategy question is whether the building can run from the main directly or needs storage and boosting, and the answer comes from a survey and calculation, not a rule of thumb. Getting it wrong is felt every day; getting it right is invisible.
How is hot water handled in a house with many bathrooms?+
It starts with an honest demand assessment: how many outlets, how they are likely to be used together, and what that means for generation and storage – all determined through calculation rather than habit. Secondary circulation is then designed so that distant bathrooms run hot promptly instead of wasting water while the tap runs cold. The generation strategy is coordinated with the heating design, because the two share plant and often share energy sources.
Can drainage noise be designed out?+
To a very large degree, yes – but only at design stage. The main tools are routing, so that stacks and wastes avoid bedrooms and quiet rooms altogether, followed by pipework selection, acoustic treatment and care over how pipes are fixed to the structure. Once a stack is built into the wrong wall, the remedial options are limited and disruptive, which is why we treat drainage acoustics as a routing decision first.
What does public health design involve for a spa or steam room?+
Wellness spaces concentrate demand: showers and drench points with high flow expectations, floor drainage that must be planned into the structure, steam generation with its own water and drainage needs, and humidity that the ventilation design must deal with. The specialist pool or spa contractor designs their own plant; our role is to make sure the water supplies, drainage capacity and plant-space provision around them are engineered rather than assumed, and that every interface is agreed on drawings.
What deliverables does a public health design include?+
Schematics and layout drawings, pipe-sizing and demand calculations, plant-room layouts, sanitaryware and equipment schedules, builders' work information and a clear statement of commissioning requirements. The intent is that a contractor can price and build from the package without guesswork, and that commissioning can be witnessed against stated criteria rather than opinion.
